Job Summary:

Americana Health and Rehabilitation Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a key member of our healthcare team, you will provide high-quality nursing care to our residents and work collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Essential Functions:
  • Supervise and coordinate the delivery of nursing care to residents, ensuring that their needs are met and their dignity is respected.
  • Comply with all relevant state and federal regulations, maintaining the highest standards of professional nursing practice.
  • Collaborate with members of the healthcare team to develop and implement individualized care plans that address the unique needs of each resident.
  • Maintain accurate and detailed records of resident care, including progress notes and medication administration records.
Knowledge, Skills and Abilities:
  • Graduate of an accredited school of nursing with a current licensure in the state of employment.
  • Demonstrated knowledge of the nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
