Hospice LPN Position

Masonicare Home Health & Hospice is seeking a skilled Hospice LPN to provide high-quality care to patients in the East Lyme, CT area.

Job Description:

• Coordinate care with community and agency resources, working closely with the designated registered nurse.
• Provide skilled nursing care, documenting clinical activities according to agency policy.
• Recognize and interpret symptoms, taking remedial measures within the scope of a Licensed Practical Nurse in CT, and reporting significant changes to the primary RN Case Manager/Clinical Manager.
• Administer medication and treatments as ordered by the physician, with the exception of IV push medication.
• Reinforce interventions, medications, effects, and side effects to patients, caregivers, volunteers, and others as directed by the RN Case Manager.
• Implement care to achieve outcomes of comfort, symptom management, safe dying, self-determined life closure, and effective grieving for patients and families.
• Maintain the dignity of the dying patient and support their unique spiritual and cultural beliefs.
• Provide holistic, family-centered care across treatment settings to improve the quality of life.
• Consult and collaborate with the interdisciplinary team and others involved in patient care.
• Provide care consistent with National Hospice and Palliative Care Organization standards of practice for hospice programs.

• Graduate of a NLN accredited school of nursing required.
• Experience in home health care or hospice preferred.
• LPN license and current drivers' license and auto insurance required.
