About the Role

Pinnacle Palliative Healthcare Nursing Services is seeking a skilled and compassionate Palliative Care Nurse Practitioner to join our team in a full-time capacity. As a Palliative Care Nurse Practitioner, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Responsibilities
  • Provide skilled nursing assessment, planning, and provision of patient care to maximize symptom palliation and determine eligibility for care.
  • Identify patient or family needs and provide supportive care and appropriate services in accordance with the attending physician's orders, the interdisciplinary team plan of care, and the policies and procedures of the agency.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Conduct regular assessments and evaluations to ensure the effectiveness of care plans and make adjustments as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and transitions.
Requirements
  • Licensed as a Nurse Practitioner/Advanced Practice Nurse.
  • Experience in hospice and/or palliative care is preferred but not required.
  • Ability to work full-time hours and maintain a flexible schedule as needed.
  • Current and valid CPR certification, Driver's License, and auto insurance.
