Job Summary:
As a skilled physical therapist with Kaiser Permanente, you will provide high-quality, cost-effective care to patients in their homes, working collaboratively with an interdisciplinary team to achieve optimal health outcomes.

This role requires a strong understanding of physical therapy principles and practices, as well as excellent communication and interpersonal skills to effectively coordinate care with patients, families, and healthcare providers.


Key Responsibilities:


Develop and implement individualized care plans that address patients' physical therapy needs, incorporating evidence-based practices and considering factors such as mobility, safety, and equipment requirements.

Collaborate with nursing, social work, and other healthcare professionals to ensure seamless care coordination and effective utilization of resources.

Supervise physical therapy assistants and home health aides, providing guidance and support to ensure high-quality care delivery.

Participate in quality improvement initiatives, contributing to the development and implementation of strategies to enhance clinical competency, regulatory compliance, and patient outcomes.

Stay up-to-date with industry developments and best practices, applying knowledge to improve patient care and departmental performance.


Requirements:
1. Bachelor's degree in Physical Therapy or a related field.
2. 1+ year of experience as a physical therapist, preferably in home care or a similar setting.
3. Current licensure as a physical therapist in the state of Washington and Oregon.
4. Basic Life Support certification.
5. National Provider Identifier.
6. Reliable automobile for travel to patient homes.
7. Excellent communication, interpersonal, and problem-solving skills.
8. Ability to work autonomously and manage a caseload effectively.
9. Familiarity with managed care industry, palliative care, and care for the chronically ill and frail elderly.
10. Proficiency in computerized clinical documentation and office application systems.
