Speech-Language Pathologist Opportunity

We are seeking a skilled Speech-Language Pathologist to join our team at Creative Solutions in Healthcare. As a PRN Speech-Language Pathologist, you will have the opportunity to work with our residents in Lancaster, TX, providing person-centered care to achieve positive outcomes.

Responsibilities:
  • Evaluate residents' conditions for swallowing or cognitive deficits and develop individualized treatment plans to increase client independence and functioning.
  • Provide skilled, medically necessary speech and language therapy assessments and implement treatment plans to address cognitive and swallowing disorders.
  • Offer education and training to residents, family members, caregivers, and other professionals on augmentative and alternative communication (AAC) systems and other relevant topics.
  • Collaborate with other professionals for diagnostic swallowing procedures as ordered by physicians and provide accurate and timely billing records and supporting clinical documentation.
  • Communicate changes in residents' conditions and respond to family requests and concerns in a timely manner.
  • Coordinate and provide training with Restorative Nursing and other Care staff to ensure high-quality care.
  • Ensure resident abuse or neglect does not occur and maintain confidentiality and safeguards for all patient information.
  • Comply with federal, state, and local legal and professional requirements.
Requirements:
  • Active/Valid Texas license in Speech-Language Pathology and good standing with all regulatory agencies and licensing boards.
  • SNF/Long-term care experience preferred, along with current knowledge of treatment practices and Net Health Electronic Documentation System.
  • Ability to manage patients with different personalities and provide effective communication and education.
