Hospice Nurse

Cherokee Regional Medical Center seeks a skilled Hospice RN to oversee the treatment of hospice patients by assessing, planning, and coordinating hospice services in collaboration with the physician, interdisciplinary group, and the patient/family/caregivers.

• Provide skilled nursing care, education, and support to maximize comfort and health of patients and their families.
• Coordinate care between healthcare providers and hospice interdisciplinary group.
• Ensure the plan of care is individualized, accurate and appropriate for the needs of the patient.
• Revises the plan of care as needed and re-evaluates the patient's health care needs regularly.
• Document skilled nursing visits at point of care and other appropriate clinical notes on the electronic medical record (EMR).
• Participates and provides required documentation for IDG meetings and care conferences.
• Educates the patient and family on diagnoses, disease process, disease progression, and treatments ordered by the provider.
• Performs diagnostic tests as ordered and obtains laboratory specimens when indicated.
• Provides in-services to staff, assists with orientation of hospice personnel and volunteers, including training of hospice aide skills and observing competence of aide skills.

• Associate Degree in Nursing or Bachelors of Science in Nursing
• Current Registered Nurse Licensure, State of Iowa
• Current and valid Iowa Driver's License
• 1+ years nursing clinical experience
• Hospice nursing experience

• Excellent written and verbal communication skills
• Highly developed problem-solving skills
