About the Role
We are seeking a highly skilled RN Clinical Manager to join our team at CenterWell Home Health. As a key member of our leadership team, you will be responsible for overseeing the clinical operations of our home health services, ensuring the highest quality of care for our patients.
Key Responsibilities
- Develop and implement clinical operations strategies to improve patient outcomes and satisfaction.
- Lead a team of clinicians to provide high-quality patient care, including assessment, planning, implementation, and evaluation of care plans.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination.
- Monitor and analyze clinical data to identify areas for improvement and implement evidence-based practices.
- Develop and maintain relationships with referral sources, community partners, and other stakeholders to promote our services and build partnerships.
- Stay up-to-date with industry trends, best practices, and regulatory requirements to ensure compliance and excellence in patient care.
Requirements
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years of experience as a Registered Nurse, with at least one year of management experience in a home care or hospice environment.
- Home care experience is required, preferably home health.
- Management and people leadership experience is required.
- OASIS experience is required.
- OASIS certification (HCS-O, COQS, and/or COS-C) is highly desired.
- Homecare Homebase (HCHB) experience is required.
- CMS PDGM billing knowledge and/or experience is required.
