Job Opportunity: Occupational Therapist

Job Summary: We are seeking a skilled Occupational Therapist to provide comprehensive therapy services in patients' residences.

Key Responsibilities:
  • Evaluate, plan, and administer occupational therapy modalities to patients in their place of residence.
  • Perform accurate and comprehensive assessments.
  • Communicate changes in patients' clinical status to healthcare team members and Agency office staff.
  • Coordinate care planning with physicians, patients, families, and healthcare team members.
  • Deliver occupational therapy modalities appropriate to the patients' clinical status and needs.
  • Educate patients, families, and caregivers in appropriate occupational therapy modalities.
  • Synthesize data from physical examinations to make clinical judgments regarding patient management.
  • Identify and address risks to patient safety and implement interventions to reduce those risks.
  • Assess the need for durable medical equipment and adaptive equipment.
  • Develop home exercise programs tailored to the patients' clinical status, needs, and age.
Professional Requirements:
  • Valid OT license in good standing with the State.
  • Graduate of an accredited Occupational Therapy curriculum.
  • Two years of work experience preferred, with prior home healthcare experience preferred.
  • Current BCLS certification.
  • Current health certificate/physical examination and TB testing.
  • Ability to relate positively and favorably with patients and staff.
  • Excellent oral and written communication skills, along with good documentation skills.
  • Knowledge of occupational therapy modalities and scope of occupational therapy practice in the State of practice.
  • Flexible, organized, and able to exercise sound judgment.
  • Effective communication in English; additional languages preferred.

  • Complete annual education requirements.
  • Maintain patient confidentiality at all times.
  • Demonstrate effective time management and organizational skills.
  • Attend staff meetings and participate in departmental in-services.
  • Ensure compliance with policies and procedures regarding department operations, safety, and infection control.
  • Participate in the organization's performance improvement and continuous quality improvement (CQI) activities.
  • Practice in accordance with the professional and Agency code of conduct.
