Job Overview

This is a direct service position at Crusader Community Health providing services in clinics, communities, and within the Integrated Primary Care model.

The Mental Health Case Manager works with patients (child, adolescent, adult) who require behavioral health support, including mental health, substance use, and psychosocial issues. Collaborating with healthcare providers, patients, families, specialists, community entities, and managed care organizations.

Responsibilities
  • Provide comprehensive case management, behavioral health counseling, education, patient and family skills training, health promotion, transitional care, advocacy, linkage, coordination, collaboration, and referrals to community services and agencies.
Requirements
  • Bachelor's degree from an accredited college/university in the human services field required.
  • Master's degree in Social Work, Counseling, Psychology, or Human Service related field preferred.
  • Certified Alcohol and Other Drug Counselor (CADC) certification or successful completion of CADC certification preferred.
  • Previous experience working with populations with substance use disorders required.
  • Previous experience working with populations with mental health issues required.
  • Case management/care management experience preferred.
  • Previous experience in ambulatory care preferred.
  • Electronic Health Record experience preferred.
What We Offer

We provide a competitive salary range estimated at $$62,000 - $80,000 per year, depending on qualifications and experience. Additionally, our comprehensive benefits package includes:
