Job Title: Physical Therapist

Join HealthPRO Heritage, a leading provider of rehabilitation services, as a Physical Therapist in our South Setauket, NY location. We are seeking a compassionate and ambitious individual to join our team and make a difference in the lives of our residents.

About the Role:

As a Physical Therapist, you will evaluate and treat residents under the orders of a physician, using a variety of therapeutic procedures to restore loss of muscle strength, coordination, and body alignment, relieve pain, and improve movement and function.

Responsibilities:
  1. Conduct thorough functional assessments of residents, utilizing standardized procedures, and develop individualized treatment plans based on resident needs and containing achievable functional goals.
  2. Comply with evaluation, treatment, and documentation guidelines, and maintain timely and accurate department records.
  3. Enter progress notes in the resident record following each treatment session, and revise treatment plans as needed throughout the treatment process.
  4. Orient residents to program services and treatment procedures, inform residents of potential risks during procedures, and work with nursing staff, residents, and families to maximize resident use of functional abilities.
  5. Provide instruction to Assistants in treatment procedures, and supervise while such staff members are working with residents.
  6. Document the course of treatment in a clear and concise discharge summary, utilizing a prescribed format.
  7. Attend and participate in rehabilitation team conferences, and provide information regarding resident progress, problems, or needs.
  8. Attend and participate in other department/facility meetings, as required.
  9. Access continuing education opportunities to maintain clinical competency, and participate in the quality improvement process.
  10. Promote the programs and services of HealthPRO Heritage through formal and informal interactions with the community.
  11. Act in compliance with HealthPRO Heritage's regulatory and professional standards and guidelines.
Qualifications:
  1. Bachelor's or Master's degree in Physical Therapy.
  2. Current license in the state of practice.
  3. Prefer supervision, training, and/or experience in a setting serving the same age/type of resident population served by this facility.
  4. Certification, if applicable, and demonstrated competency in identified areas of specialization.
  5. Excellent oral and written communication and interpersonal skills.
  6. Demonstrated competency in applying the principles, methods, materials, and equipment used in physical therapy.
  7. Ability to assess resident needs and develop and implement a comprehensive plan of care.
  8. Knowledge of accreditation standards and compliance requirements.
