LPN for Home Health

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Beacon Care. As a home health care provider, we offer a unique opportunity for nurses to provide high-quality patient care in a flexible and supportive environment.
Key Responsibilities- Participate in the planning and coordination of total patient care in conjunction with the Registered Nurse and physician's plan of care.
- Accurately report and record the patient's condition and care, including signs and symptoms that may indicate a change.
- Prepare clinical/progress notes and summaries.
- Assist patients with activities of daily living while teaching appropriate self-care techniques.
- Provide and maintain a safe environment for patients.
- Assist physicians and Registered Nurses in complex nursing situations.
- Follow universal precautions when giving any aspect of patient care.
- Follow emergency procedures in the event of any catastrophic event, incident, or significant change in the patient's condition.
- Tracheotomy experience is required.
- One year of experience as an LPN in a home care setting is preferred.
- Current state license as an LPN is required.
- Graduate of an accredited program of practical nursing is required.
- Proof of current CPR certification is required.
- Ability to work a flexible schedule is essential.
- Excellent interpersonal skills are required.
- Valid driver's license and reliable transportation are required.
- Ability to work a flexible schedule and travel locally is required.
