As a key member of our leadership team, you will be responsible for managing and directing the MDS assessment process, ensuring compliance with RAI guidelines and optimizing case mix scores.

Responsibilities:

* Identify Medicare entitlement and eligibility for prospective residents
* Complete weekly chart audits to assess documentation support for skilled Medicare coverage
* Monitor MDS documentation and charting requirements that support services provided to meeting billing requirements
* Chart MDS and pertinent narrative data to optimize case mix scores
* Conduct daily assessments of Medicare/patients and coordinate therapy, care mapping and discharge planning
* Conduct weekly Medicare/case management meetings to review plan of care
* Perform other tasks as assigned

Requirements:

* Graduate of an accredited School of Nursing, BS preferred
* RAC certified
* Valid RN license in the state employed
* Three years of nursing experience in a long term care environment preferred
* MDS coordinator experience
