Speech-Language Pathologist Job Description

Hillcrest Health Care Center in Hawarden, IA, is a skilled nursing facility offering long-term care, post-acute rehab, and geriatric outpatient therapy services.

Key Responsibilities:
  • Evaluate patients with communication, cognitive, or swallowing disorders and develop individualized plans of care.
  • Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with communication, cognitive, and/or swallowing disorders.
  • Evaluate treatment outcomes and modify services as needed.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
  • Document all required information in the electronic documentation system and maintain patient records according to company policy.

  • Licensed Speech-Language Pathologist
