About the Role

We are seeking a skilled and compassionate LPN to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide one-on-one care to our patients in their homes, helping them to recover from illness or injury and maintain their independence.

Responsibilities
  • Implement the physician's prescribed plan of care, ensuring that our patients receive the highest level of care possible.
  • Conduct patient assessments and coordinate care with other healthcare professionals to ensure seamless transitions and optimal outcomes.
  • Administer prescribed medication, treatments, and therapies as ordered by the physician.
  • Document all care provided in a timely and compliant manner, using our electronic documentation system.
  • Teach and train family members on health promotion and disease prevention strategies.
  • Accept and write verbal orders from physicians as required, and obtain co-signature from the Case Manager on all verbal orders.
  • Follow up with physicians, clients, family members, staff, and Case Managers on client problems, concerns, and changes in client condition.
Requirements
  • Active, unencumbered LPN license in the state where you will practice.
  • Current CPR certification from the American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within the last 12 months).
  • At least six months of prior hands-on nursing experience (preferred but not required).
  • Reliable transportation.
