Licensed Practical Nurse

Premise Health is a leading direct healthcare provider seeking a skilled Licensed Practical Nurse to join our team in an ambulatory care setting. As a key member of our healthcare team, you will provide high-quality patient care and services in a fast-paced, dynamic environment.
Key Responsibilities- Conduct thorough patient interviews to determine medical problems and document findings in patient charts.
- Obtain and record patient vital signs and weight, as well as assist physicians with patient examinations as needed.
- Review physician orders, lab requests, and follow-up needs with patients, and document services performed on appropriate forms.
- Administer medication, apply dressings, and use clinic equipment as instructed.
- Collect patient samples for testing, perform routine laboratory tests, and sterilize equipment and supplies.
- Respond to patient inquiries, follow up on missed appointments, and coordinate referrals through insurance and other physician offices.
- Graduate of a National League for Nursing accredited program with an Associate's degree (A.S.) or equivalent from a two-year college or technical school.
- Licensed as a Practical or Vocational Nurse.
- Current hands-on Certification in Basic Life Support, with Advanced Cardiac Life Support may also be required based on scope of contract services.
- At least three years of clinical experience as a LPN/LVN in an ambulatory care, occupational health, or emergency department setting.
- Knowledge of workplace health and safety concepts and OSHA regulations.
- Strong computer skills, including Internet software, spreadsheet, and word processing.
- Knowledge and experience with Electronic Medical Records.
