Oak Street Health is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a key member of our primary care team, you will provide high-quality, patient-centered care to our Medicare patients.
Responsibilities:
- Provide routine and acute care to patients, including conducting office visits, administering annual wellness visits, and health risk assessments.
- Collaborate with other healthcare providers, specialists, and agencies to ensure comprehensive care.
- Lead population health initiatives, including coordinating care for patients with complex needs.
- Assist with phone triage and outreach, and educate patients on their health conditions and care plans.
Requirements:
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner.
- Active, non-probationary state Nurse Practitioner license.
- Active DEA license.
