Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Mobile Crisis Team. As an LPN, you will play a vital role in providing crisis intervention and stabilization services to individuals in need. Your expertise will be instrumental in assessing clients, developing treatment plans, and collaborating with other healthcare professionals to ensure the best possible outcomes.

Key Responsibilities
  • Conduct thorough nursing evaluations at the site of the encounter, including medical history, current medical concerns, medications, and recent hospitalization.
  • Collaborate with clients' current mental health resources, therapists, and other members of the Interdisciplinary Team (IDT) to plan and implement treatment plans.
  • Present pertinent data obtained through direct observation to the IDT, including client interview, physical evaluation, and other reports as received by external resources.
  • Administer and document medications given, ensure necessary consents have been completed and documented prior to administration.
  • Document all behavioral and physical changes in the client's status and other pertinent information in the nursing progress notes and in the electronic health records.
Requirements
  • Licensed as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
  • One (1) year of psychiatric, medical surgical or chemical dependency nursing experience.
  • Knowledge and understanding of program regulating bodies (e.g. Office of Mental Health & Substance Abuse Services OMHSAS, Department of Behavioral Health) preferred.
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities.
  • Demonstrated strong attention to detail, time management and organizational skills.
  • Current CPR certification; must be able to perform CPR, first aid, crisis intervention, medication administration, and crisis intervention techniques using agency-trained protocols.
Working Conditions

The LPN will work in a fast-paced environment, managing and prioritizing multiple, often competing, priorities. The ability to work effectively as part of a team and maintain confidentiality of client records is essential. The LPN will be required to perform CPR, first aid, crisis intervention, medication administration, and crisis intervention techniques using agency-trained protocols.
