About the Role

AdvisaCare, a leading home healthcare provider, is seeking a skilled Home Health Occupational Therapist to join our team on a PRN basis. As a Home Health Occupational Therapist, you will play a vital role in helping patients regain their independence and maintain safety with their daily living activities.

Key Responsibilities
  • Evaluate patients' functional abilities and limitations, including their home environment, to determine their needs and goals for therapy.
  • Develop individualized treatment plans to improve patients' physical, cognitive, and psychosocial function, and track their progress towards goals.
  • Utilize therapeutic exercises and activities, adaptive equipment, and home modifications to help patients regain or maintain their independence in daily living activities.
  • Educate patients and caregivers on how to safely perform daily living activities and use specialized equipment.
  • Communicate with physicians and other healthcare professionals to coordinate care plans, and make referrals for other services as needed.
  • Maintain accurate and up-to-date documentation to meet regulatory and insurance requirements.
  • Participate in team meetings to optimize patient outcomes.
Requirements
  • Current license to practice as an Occupational Therapist in the state of Michigan.
  • Degree in Occupational Therapy from an accredited program.
  • Minimum of 1 year of experience in home health care is preferred.
  • Strong communication and interpersonal skills to collaborate with patients, caregivers, and other healthcare professionals.
  • Ability to work independently and manage a caseload efficiently.
  • Basic computer skills to document patient care accurately.
  • Reliable transportation and a valid driver's license, as travel to patients' homes is required.
