Job Summary

We are seeking a skilled Case Planner to join our team at The Salvation Army Eastern Territory. The successful candidate will provide case planning and coordination services for youth and their families, stabilizing housing, assisting with skill development, and reducing the risk of placement in the foster care system.

This is a full-time, hourly position with access to our full benefits program. The ideal candidate will have an Associate's degree with five years of experience working with families or a Bachelor's degree in human services with a minimum of three years of experience.

The Case Planner will engage and maintain regular contact with assigned clients, conducting regular home visits and providing casework/counseling, crisis intervention, and brokerage and networking advocacy. The successful candidate will also coordinate skill building activities and participate in weekly supervision and team case conferences.

Responsibilities
  • Provide case planning and coordination services for youth and their families.
  • Conduct regular home visits and engage with assigned clients.
  • Provide casework/counseling, crisis intervention, and brokerage and networking advocacy.
  • Coordinate skill building activities and provide referrals for auxiliary services as needed.
  • Participate in weekly supervision and team case conferences.
Qualifications
  • Associate's degree with five years of experience working with families or a Bachelor's degree in human services with a minimum of three years of experience.
  • Valid NYS Driver License and access to private transportation during work hours.
  • Computer proficiency, including word processing skills and the ability to provide culturally competent practice.
