Job Summary

We are looking for a skilled Nurse Practitioner with experience in primary care and geriatric medicine to join our team. The ideal candidate will have a strong background in assessing patients' medical and mental health needs, developing and implementing plans of care, and collaborating with healthcare teams to improve patient outcomes.

Responsibilities
  • Assess patients' medical and mental health needs and develop plans of care to proactively manage their conditions and address barriers to care
  • Ensure compliance with local, state, and federal agencies related to clinical services
  • Prescribe medications and order lab work, diagnostic procedures, and consultations as needed
  • Monitor patients' compliance and response to treatment and modify plans as necessary
  • Collaborate with RN case managers as part of an integrated care team
Requirements
  • FNP, AGNP, APRN, or PA license required
  • Degree from an accredited APRN or PA program
  • Experience in clinical nursing or rehab in geriatric populations
  • Electronic Health Records experience
  • Working knowledge of Microsoft applications, including Word, Outlook, and Excel
Preferred Qualifications
  • Primary care experience in geriatric medicine
  • 3 years of experience in long-term care
