Key Responsibilities:
  • Evaluate the needs of our members and develop personalized care plans to meet their unique needs.

  • Coordinate with community resources to ensure that our members have access to the services they need.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  • Provide education and support to our members and their families to help them navigate the healthcare system.
Requirements:

We are looking for a highly skilled and compassionate LTSS Service Care Manager who has:

  • A Master's degree in Mental Health or Social Work or a Graduate degree from an Accredited School of Nursing.
  • 2-4 years of experience in case management or a related field.
  • Licensed Behavioral Health Professional or RN with psychiatric experience based on state contract requirements.
Preferred Qualifications:

We prefer candidates with:

  • Master's level un-restricted licensed behavioral health clinician; LCSW, LPC, or Psych/Mental Health RN.
  • 3+ years of case management experience with the adult population with serious mental illness and behavioral health conditions or special needs.
  • 3+ years of experience in patient advocacy or coordinating community mental health and/or personal assistance or social services for members.
