Visiting LVN

We are seeking an experienced Hospice LVN to join our team at ACT Medical Group. As a Visiting LVN, you will play a vital role in providing high-quality, compassionate care to our patients facing a life-limiting illness.

• Contribute to the provision of hospice care, working in collaboration with our interdisciplinary team to develop and implement individualized care plans.
• Provide direct skilled bedside nursing care, ensuring the physical and emotional needs of our patients are met.
• Evaluate patient and family needs, identifying opportunities for improvement and implementing effective solutions.
• Perform selected acts in the care of the terminally ill, working under the direct supervision of a registered nurse.

• Licensed as a Vocational Nurse in the State of California.
• Ability to read, write, speak, and understand English and Spanish.
• Valid driver's license, auto insurance, and reliable transportation.
