Key Responsibilities

  • Conduct comprehensive assessments of member needs and develop personalized care plans to address physical, behavioral, and social determinants of health.">
  • Collaborate with healthcare providers, community resources, and other stakeholders to ensure seamless coordination of care and services.">
  • Provide education and support to members on healthy behaviors, disease management, and self-care strategies.">
  • Facilitate connections to community resources, including housing, transportation, and food assistance.">
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.">

Requirements

To be successful in this role, you will need:

  • A current Registered Nurse license in the state of Delaware.">
  • 2+ years of experience in case management, discharge planning, or a related field.">
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.">
  • Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.">
  • Proficiency in electronic documentation systems and Microsoft Office applications.">
  • Reliable transportation and flexibility to travel throughout the state.">
