Key Responsibilities- Provide personal care, including bathing, back rubs, oral hygiene, and changing bed linens
- Assist with dressing and undressing patients
- Plan and prepare nutritious meals, including shopping and feeding patients
- Observe and report on patient skin health to prevent breakdown
- Communicate with the Case Manager regarding patient condition and significant changes
- A supportive and compassionate team environment
- Mileage reimbursement for travel to patient locations
- Employee Referral Program and Employee Discount Programs
- Current CNA certification
- Hospice experience preferred
- Ability to read and follow written instructions and document care given
- Possesses current CPR certification
- Able to lift 50 pounds
- Must be a licensed driver with an insured vehicle
