About the Role:

  • Collaborate with experts like physicians, nurse practitioners, behavioral health providers, and social workers to create tailored care plans, addressing patients' unique needs and challenges.
  • Deliver comprehensive support across various modalities, including video, phone, and in-person interactions, ensuring seamless patient care and effective communication with the care team.
  • Operate as Galileo's eyes, ears, and hands in the field, providing clinical support such as taking vitals, performing phlebotomy, facilitating video visits, and administering medications/vaccines within your scope of practice.

Your Responsibilities:

  • Establish strong relationships with patients and families, addressing their concerns and needs through empathetic and effective communication.
  • Work collaboratively with the care team to develop and implement individualized care plans, driving positive health outcomes and patient satisfaction.
  • Provide expert clinical support, ensuring patients receive timely and appropriate care, and maintaining open communication with the care team.

Requirements:

  • 1-2 years of experience as an LPN, Paramedic, or CMA.
  • Experience with phlebotomy (or willingness to learn) and injections (vaccines) is a plus.
  • Reliable access to personal transportation and ability to lift up to 20 pounds.
  • Fully vaccinated with all necessary vaccinations, including a COVID-19 booster.
