**Key Responsibilities:**

* Conduct in-home and telehealth visits for patients in our service area
* Provide primary medical care and palliative care for chronic and acute illnesses
* Perform physical exams, diagnose and treat acute and chronic diseases
* Interpret laboratory and test results
* Prescribe and manage medications and other therapies

**Requirements:**

* Current Louisiana license required
* Excellent communication and problem-solving skills
* Experience in hospice and/or home health care
* Ability to work independently and as part of a team
