- Develop and implement care plans that are goal-oriented and patient-centered, consistent with available resources.
- Collaborate with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
- Current license to practice as an LPN in the State of Delaware.
- Associates degree or completed an accredited LPN education program.
- Work experience in an acute or chronic care facility, or home health agency is preferred.
