Your Role

As a Hospice Medical Social Worker, you will be responsible for providing direct patient counseling services, supporting crisis interventions as needed, and assisting patients and their families in community resource planning and access. You will also help patients and their families navigate their ongoing healthcare journey, providing emotional support and guidance throughout the process.

Requirements
  • Education - A Master of Social Work (MSW) degree is required.
  • Licensure - A current, unrestricted license to practice as a Licensed Social Worker (LSW) in the state associated with this position is required; a Licensed Independent Social Worker (LISW) is preferred.
  • Experience - One year of experience practicing as a Licensed Social Worker in a hospice/palliative care or similar setting is preferred.
  • Reliable Transportation - Reliable transportation to and from care sites or work locations is required.
