Job Summary

We are seeking a compassionate and experienced Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our multidisciplinary team, you will provide high-quality care to terminally ill patients and their families, promoting their physical, emotional, and social well-being.

 

Responsibilities

  • Conduct comprehensive assessments of patients' physical and psychosocial needs, identifying areas for intervention and developing effective care plans.
  • Manage pain effectively, educating patients and their families on pain management techniques and strategies.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination.
  • Prioritize patient safety, adhering to infection control protocols and ensuring a clean and safe environment.
  • Maintain accurate records of patient interactions, treatment plans, and outcomes, reporting any concerns or issues promptly.
  • Participate in case conferences to discuss patient progress, pain management, bereavement counseling, and other relevant topics.

 

Requirements

  • A minimum of 1 year of nursing experience, preferably in hospice care.
  • A valid Registered Nurse license in the State of Indiana.
  • A reliable vehicle, current driver's license, and adequate insurance coverage.

Location

This position is based in South Marion County and surrounding communities, with regular travel required to visit patients in their homes.
