As a Home Health Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities. Our team is committed to providing personalized care and support to our patients, and we are seeking a skilled and compassionate Physical Therapist to join our team.

  • Develop and implement individualized physical therapy treatment plans to help patients achieve their goals and improve their overall health and well-being.
  • Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop targeted interventions to address their needs.
  • Collaborate with our interdisciplinary team, including nurses, occupational therapists, and speech-language pathologists, to ensure comprehensive care and support for our patients.
  • Provide education and training to patients and their families on proper techniques and strategies for managing their conditions and promoting healthy lifestyles.
  • Document patient progress and outcomes, and participate in regular team meetings to discuss patient care and make adjustments to treatment plans as needed.

Requirements

To be considered for this role, you must have a degree from an accredited Physical Therapy program and a current and unrestricted Physical Therapy license. You should also have a minimum of one year of experience in physical therapy, preferably in a home health setting. Strong communication and organizational skills are essential for this role, as well as a valid driver's license and reliable transportation.
