School Based Licensed Practical Nurse (LPN) Job Description

Meridian Health Services is seeking a dedicated and compassionate School Based Licensed Practical Nurse (LPN) to join our team. As a School Based LPN, you will play a vital role in providing high-quality healthcare services to students in our School Based Health Clinic located in Kokomo, IN.

Key Responsibilities:
  • Administer medications as ordered by medical staff, ensuring accurate and timely documentation in the Medication Administration Record (MAR).
  • Provide health screenings and direct students to appropriate integrated medical care services, including referrals to emergency rooms, physician offices, and pharmacies.
  • Collaborate with medical staff to review lab and test results, reporting concerns and abnormal results in a timely manner.
  • Obtain medical histories and perform physical assessments as directed by medical staff, including vital signs and other point-of-care testing.
  • Participate in treatment planning and reviews, ensuring accurate and timely documentation of student health records.
  • Function as a liaison with students, caregivers, and emergency services to clarify medication orders, obtain prescriptions, and provide referrals.
Requirements:
  • Must be a Licensed Practical Nurse with a current Indiana license.
  • Must have successfully completed an Accredited Nursing Program.
