Job Summary

We are seeking a highly skilled and compassionate Care Team Staff to join our team at Tippecanoe County. As a key member of our care team, you will play a vital role in providing exceptional support services to our residents.

Key Responsibilities
  • Provide reminders and observe residents during medication self-administration at scheduled times, taking vital signs and assisting residents with treatment self-administration according to physicians' orders, recording on residents' charts as required.
  • Monitor residents' health status and communicate any changes to the care team coordinator.
  • Notify supervisor, residents' physician or house physician of resident health status changes as needed and notify emergency personnel when the need arises.
  • Schedule and prepare residents for medical appointments, including preparing forms, arranging transportation, and charting and ordering resulting medications from pharmacy as needed.
  • Weigh and take blood pressure of residents weekly or monthly as ordered by physician and enter on charts.
  • Schedule laboratory work monthly or as ordered and prepare required forms.
  • Communicate collaboratively with all other staff in department regarding changes in medication, treatment, and various record changes, and changes in residents' condition.
  • Order medication refills from pharmacies as needed.
  • Perform duties of management when they are not in the building. Notify Director or Assistant director of any issues needing immediate attention.
  • Chart any health status changes of residents in their charts and notify appropriate Doctors, therapist, or medical personnel.
  • Complete monthly summary reports on all residents as required.
  • Periodically admit new residents as needed, including reviewing physicians' orders, preparing charts, and checking/charting vital signs.
  • Periodically supervise and assist residents with bathing as needed, including providing reminders and assistance regarding personal/oral hygiene.
  • Make rounds to monitor residents as scheduled throughout the night.
  • Answer telephone as needed, directing to appropriate personnel or taking messages.
  • Monitor medications and supplies inventory and order from pharmacy.
  • Perform cleaning chores as scheduled, such as washing/cleaning medication trays, countertops, medicine carts, bathroom, refrigerator, desk, and charts.
  • Occasionally transport residents to and from appointments.
  • Perform cleaning chores as needed, such as cleaning spills.
  • Serve on 24-hour call for emergencies.
  • Perform related duties as assigned.
Requirements
  • High school diploma or GED.
  • Possession of valid Indiana Qualified Medical Aide license, Core A&B certificate, or Medical Assistant degree or some type of experience in health care field is preferred.
  • Working knowledge of principles and practices of good hygiene, and ability to apply such knowledge in assisting residents with physical and mental disabilities.
  • Working knowledge of universal health precautions and bloodborne pathogens control, and ability to apply such knowledge to prevent infection of self and others.
  • Working knowledge of standard housekeeping procedures and ability to properly mix and use cleaning chemicals, disinfectants and related tools, such mops and brooms.
  • Ability to effectively communicate orally and in writing in a tactful, courteous, and positive manner with co-workers, other County departments, residents, families, and physicians, including being sensitive to professional ethics, gender, cultural diversities, and disabilities.
  • Ability to provide public access to or maintain confidentiality of department information and records according to State requirements.
  • Ability to comply with all employer and department policies and work rules, including, but not limited to, attendance, safety, drug-free workplace and personal conduct.
  • Ability to understand, memorize, retain, and carry out written and oral instructions, and present findings in oral or written form.
  • Ability to work alone and with others in a team environment with minimum supervision, often under time pressure.
  • Ability to regularly work weekend hours and occasionally work extended and evening hours.
  • Ability to serve on 24-hour call and respond swiftly, rationally and decisively to emergency situations.
  • Ability to supervise and instruct Resident care/environmental and Dietary personnel in absence of supervisor.
Responsibility

Incumbent performs standard, recurring duties according to well-established guidelines. Incumbent receives direct and indirect supervision, with work periodically reviewed upon completion or during critical phases. Errors in incumbent's work are usually prevented and detected through procedural safeguards or supervisory review. Errors in medications reminders could result in endangerment to residents.

Personal Work Relationships

Incumbent maintains communication with co-workers, residents, families, and physicians for purposes of exchanging information, and providing support services care to disabled residents.

Physical Effort and Work Environment

Incumbent performs duties in a residential care facility for persons with physical and mental disorders, involving sitting and walking at will, walking/standing for long periods, hearing sounds/communication, handling/grasping/fingering objects, bending, occasionally pushing/pulling/lifting/carrying objects weighing more than 50 pounds, and exposure to minimum hazards associated with body fluids and standard cleaning chemicals, for which universal safety and health precautions must be followed at all times to prevent infection/injury to self and others.

Incumbent occasionally works extended, and regularly works weekend and evening hours. Incumbent serves on 24-hour call for emergencies. Occasional encounters dealing with agitated residents.
