Responsibilities:- Conduct home visits to assess and develop individualized care plans for clients.
- Perform diagnostic tests and measurements to evaluate client mobility, strength, and balance.
- Develop and implement manual therapeutic exercises, gait training, and other interventions to promote client independence.
- Educate and instruct clients and their families on rehabilitative care and activities necessary for optimal health and safety.
- Accurately document client care management and services provided.
- Current license as a Physical Therapist in Massachusetts.
- Experience in home care or geriatric care preferred.
