We are seeking a highly skilled and compassionate Physical Therapist to join our team at JAG Physical Therapy. As an outpatient orthopedic specialist, you will play a crucial role in helping patients achieve optimal health and wellness.

Job Summary

The ideal candidate will possess a Bachelor's or Master's degree, a valid license, and strong clinical decision-making skills. You will be responsible for assessing patients' needs, implementing effective treatment plans, and maintaining patient confidentiality. A positive, professional demeanor and excellent communication skills are essential for success in this role.

Responsibilities
  • Collects and records patient care data, including initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communication with the healthcare team.
  • Prepares for evaluations, obtains patient history, exhibits knowledge of patient diagnoses, and researches unfamiliar diagnoses.
  • Selects and administers tests and measures, synthesizes evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
  • Establishes treatment plans based on evaluation results, goals, and best practice, collaborates with interdisciplinary teams, patients, and patient support systems.
  • Identifies need for modifications to plan of care and goals, initiates and completes discharge plans.
  • Provides education/training to patients and/or patient support systems, oversees support staff of PTA, ATC, PT aides.
Requirements
  • Bachelor's or Master's Degree required, Doctorate degree preferred.
  • Licensed Physical Therapist in the state practicing in.
  • Completed state regulated CEU requirement in appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience/knowledge with EMR software is preferred.
  • Excellent oral, written communication, strong clinical decision-making skills.
  • Basic computer skills, works as a team member and individually with minimal supervision.
