Job Overview:

Clinic Nurse Navigator

This role is designed to ensure seamless coordination of patient care, promoting effective communication among healthcare providers and enhancing the overall patient experience. The Clinic Nurse Navigator will work closely with patients, families, and healthcare teams to develop personalized care plans, addressing complex needs and challenges.

Key Accountabilities:

  • Develop and maintain strong relationships with patients, families, and healthcare providers to ensure timely and effective care coordination.
  • Conduct comprehensive assessments of patient/family psychosocial and care needs, informing the development of individualized care plans.
  • Collaborate with the medical team to identify and address gaps in care, ensuring seamless transitions and optimal patient outcomes.
  • Provide coaching and education to patients and families, empowering them to manage their health and make informed decisions.
  • Promote evidence-based practices and guidelines, contributing to the advancement of healthcare quality and safety.
  • Participate in interdisciplinary meetings and collaborations, sharing expertise and best practices to enhance patient care.

Requirements:

  • Licensure/Certification: Licensed Practical Nurse or Registered Nurse, current and unencumbered license, preferred experience in case management.

Salary Estimate: $65,000 - $85,000 per annum, based on experience and qualifications.
