POSITION SUMMARY:

The Patient Care Coordinator plays a pivotal role in the management of patients within the oncology department. This position is designed to ensure that patients receive comprehensive care tailored to their unique needs, particularly those identified as high-risk.


KEY RESPONSIBILITIES:
  • Collaborate with a multidisciplinary team to oversee patient care pathways.
  • Facilitate communication between patients and healthcare providers to enhance care coordination.
  • Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
  • Provide education and support to patients and their families regarding treatment options and resources.

QUALIFICATIONS:

To be considered for this role, candidates must possess:

  • A Bachelor of Science in Nursing (BSN) is required.
  • A minimum of 4-5 years of experience in oncology nursing is essential.
  • Experience in an outpatient care setting is preferred.
  • A valid Maryland Registered Nurse License.
  • Current CPR certification from the American Heart Association.
  • Oncology Nurse Certification is mandatory.
