Job Summary
The Inpatient Social Worker / Case Manager plays a vital role in promoting patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs.
Key Responsibilities
- Collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention.
- Develop and implement individualized care plans to address the unique needs of each patient.
- Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions.
- Identify and address social determinants of health that impact patient outcomes.
- Provide education and support to patients and families on health-related topics and community resources.
Requirements
- Master's degree in Social Work (MSW) or related field.
- 2+ years of experience in hospital social work or community social services agencies.
- LMSW required; LCSW/LCSW-C preferred.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
