Job Summary

We are seeking a skilled Speech Language Pathologist to join our team as a Home Health Speech Language Pathologist. As a member of our team, you will be responsible for providing high-quality speech therapy services to patients in their homes, assessing and treating speech, language, social communication, cognitive communication, and swallowing disorders.

Key Responsibilities
  • Conduct initial assessments and evaluations of patients to establish a plan of care, including scheduling subsequent visits as clinically appropriate.
  • Provide detailed notes and documentation regarding home therapy visits and patient progress.
  • Coordinate care by conferencing with other healthcare team members to ensure comprehensive care.
  • Observe, record, and report to the Primary Care Physician the patient's treatment, response, and progress, including significant changes.
  • Perform re-evaluations of patient progress at least every thirty (30) days and update the plan of care as necessary.
  • Teach patients and caregivers about the patient's impairments and provide support and educational materials to support interventions and foster independence.
Requirements
  • Master's degree in Speech Language Pathology.
  • Active Speech Language Pathology license (CCCs).
  • Ability to practice speech therapy unencumbered within state and federal guidelines.
  • At least one year of experience working as a Speech Language Pathologist (no CFYs).
  • Home Health and Home Care Home Base documentation experience.
