Responsibilities
- Conduct comprehensive health histories and physical assessments of patients with acute, chronic, and potential health problems.
- Collaborate with physicians to establish medical diagnoses and treatment plans.
- Provide continuity of care between clinic visits and assign specific orders within the limits of the practice agreement/scope of practice.
- Assign direction to clinic staff regarding patient care issues and participate in a team model of care delivery to identified groups/panels of patients.
- Meet regularly with physicians to effect programmatic changes and develop protocols.
- Participate in quality assessment and improvement activities, identifying important aspects of care and ensuring findings are shared to improve patient experience and practice operations.
- Work collaboratively with staff to provide ongoing patient care and management and hold responsibility for timely and accurate written documentation of patient care encounters/procedures.
Requirements
- Primary Care Practice Setting experience preferred.
- Masters in an AMA-approved NP program - FNP.
- NYS Nurse Practitioner License.
- NP Board Certification - FNP.
- BLS and ACLS certification.
