Cardiac Home Health Nurse Specialist

WCS Healthcare Partners is seeking a skilled Cardiac Home Health Nurse to join their team. As a cardiac home health nurse, you will be responsible for providing specialized care to patients with heart conditions in the comfort of their own homes.
The ideal candidate will have a valid NJ RN License, current BLS CPR certification, and a valid NJ driver's license. Additionally, they should have recent work experience in a hospital setting, preferably in cardiology or heart failure units.
Job Responsibilities- Assess patient condition and develop personalized care plans to optimize health outcomes.
- Deliver high-quality patient care through evidence-based practice and interdisciplinary collaboration.
- Rapidly identify changes in patient condition and assess knowledge gaps to foster successful independent community-based disease management.
The salary for this role is estimated to be around $45-$60 per hour, based on national averages for similar positions in New Jersey.
