Job Opportunity for Occupational Therapist

We are seeking a skilled Occupational Therapist to join our team at Gpac. The ideal candidate will provide comprehensive occupational therapy services in a Home Health setting, conducting evaluations, administering treatments, and overseeing discharges. This role involves creating and implementing individualized care plans to help patients regain optimal function following injury, illness, or physical impairment.

Key Responsibilities:
  • Conduct thorough evaluations to assess patients' functional abilities and needs.
  • Develop and implement customized treatment plans aimed at restoring maximum function.
  • Perform regular treatment sessions and adjust plans as necessary based on patient progress.
  • Oversee the discharge process, ensuring patients and their families are well-prepared for the next steps in care.
  • Supervise and collaborate with Occupational Therapy Assistants, providing guidance and support to achieve therapeutic goals.
  • Maintain detailed documentation of patient progress and treatment outcomes.
  • Communicate effectively with patients, families, and other healthcare team members to ensure coordinated care.
Qualifications:
  • Graduate from an accredited Occupational Therapy program.
  • Licensed or eligible for licensure by the Ohio OTPTAT Board.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
