Position Overview
The Registered Nurse in Home and Community-Based Services is responsible for delivering and overseeing nursing and personal care to patients in their residences, as prescribed by healthcare providers and in accordance with relevant regulations and agency policies. The RN collaborates with other healthcare professionals to ensure a seamless continuum of care by coordinating treatment plans and sharing vital information with physicians, agency staff, patients, families, case managers, and community resources.
Key Responsibilities
- Deliver high-quality clinical services in alignment with established infection control protocols and care plans, collaborating with the patient's care team from admission to discharge.
- Conduct comprehensive nursing assessments as mandated by federal and/or state program guidelines and payer requirements.
- Verify patient eligibility and medical necessity for services as per payer and agency policies.
- Develop and update personalized care plans in conjunction with other community providers, adhering to federal and/or state program regulations.
- Ensure that the frequency and duration of care plans align with patient needs, making necessary adjustments with physician consent.
Preferred Qualifications
- A minimum of one year of experience as a Registered Nurse is preferred.
Licensure Requirements
- Current RN licensure in the state of practice is required.
- A valid driver's license, vehicle insurance, and reliable transportation are necessary.
- Current CPR certification is mandatory.
