Job Title: Social Work Lead

At Tucson Medical Center, we are seeking a highly skilled and compassionate Social Work Lead to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Job Summary:

The Social Work Lead will be responsible for overseeing patients and staff, scheduling, timecard edits and approvals, annual evaluations, staff assignments, attending and facilitating meetings, and new hire orientation. You will also assist patients with environmental difficulties that interfere with obtaining maximum benefits from medical care, serve as a liaison between medical and nursing staffs, patients, relatives, and appropriate outside agencies, and interpret and assist in resolving social problems that relate to medical condition and/or hospitalization.

Key Responsibilities:
  • Interview patients and family members to obtain information about home environment, family relationships, health history, and patient's personality traits.
  • Plan, organize, direct and schedule spiritual programs and services that meet the needs of patients/families, hospital employees, and medical staff.
  • Consult with hospital staff regarding hospital ministry; collaborate with members of the hospital care team in identifying and resolving problems.
  • Act as a liaison between patient, family, and hospital staff during periods of crisis, e.g. illness, dying, lengthy or difficult surgery.
  • Assist in developing and implementing the patient's care plan.
  • Interpret nature of illness and prescribed course of treatment to patients and families; communicate family issues to other staff members; participate in planning conferences.
  • Plan for needed care with nurse practitioner, physician, patient, and family.
  • Introduce programs designed to meet the needs of the elderly after discharge and make appropriate referrals and arrangements for services.
  • Act as liaison between medical/nursing staff, patients, families, and community, arranging for post-discharge care and patient's transportation.
  • Maintain current, accurate documentation in the client's medical record.
  • Adhere to TMC organizational and department-specific safety, confidentiality, values, policies, and standards.
  • Provide supervision as required to social work staff.
  • Work collaboratively and supports the efforts of other team members.
  • Assist in monitoring staff performance, behaviors, and competency for performance reviews and reports information to manager.
  • Assign and coordinate patient flow to deliver safe and effective patient-centered care.
  • Perform other specific department tasks as assigned by members of the department's leadership team.
  • Ensure that appropriate staffing levels meet patient care needs.
  • Perform related duties as assigned.
Bereavement Program:

The Social Work Lead will also be responsible for assisting in developing and implementing the patient's/family member care plan, providing individual and group counseling, following up on critical incident briefings to doctors, ER and support staff, providing home counseling if needed, follow up telephone calls and referrals if needed, developing education component for volunteer training and annual hospice foundation teleconference, and preparing and implementing all logistics for program needs.

Requirements:
  • Master's degree in Social Work required.
  • Two (2) years of social work experience in an acute care setting preferred.
  • Two years supervisory experience preferred as required by department.
  • HeartSaver First Aid/CPR or Basic Life Support (BLS) certification required per department.
  • A valid AZ fingerprint clearance card may be required per department.
  • Licensed by the Arizona Board of Behavioral Health Examiners as LCSW may be required per department.
Knowledge, Skills and Abilities:
  • Knowledge of interviewing techniques used to garner personal and health information to better assist patients.
  • Skill in assessing socio-economic factors that might hinder a patient from getting the best care.
  • Ability to read and comprehend simple instructions, short correspondence, and memos.
  • Ability to write simple correspondence.
  • Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
  • Ability to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
