Comprehensive Care Professional

At Nonotuck Resource Associates, we are seeking a highly motivated and compassionate Comprehensive Care Professional to join our dedicated team.
The successful candidate will be responsible for providing comprehensive care and support to individuals with disabilities, ensuring their overall well-being and quality of life. This role involves conducting intake and assessments, developing Individualized Plans of Care, and collaborating with caregivers and other stakeholders to ensure seamless service delivery.
Key Responsibilities:
- Nursing Assessments and Support: Conduct thorough nursing assessments to identify the needs of individuals and develop personalized plans of care. Provide ongoing support and guidance to caregivers to ensure high-quality care.
- Supervision and Training: Supervise and train caregivers on best practices in care provision, including health and safety protocols. Develop and maintain detailed records of caregiver training and evaluation.
- Communication and Collaboration: Maintain effective communication with caregivers, families, guardians, and other stakeholders to ensure that the needs of individuals are met. Collaborate with the multidisciplinary team to address any concerns or issues related to care delivery.
