Join Our Team as a Licensed Practical Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Interim HealthCare. As a LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities:
  • Provide direct patient care according to the provider's prescribed plan of care and nursing scope of practice
  • Conduct skilled assessments and provide exceptional patient-centered care in the home
  • Develop and implement comprehensive care plans that address patient needs
  • Communicate patient conditions and collaborate with healthcare providers to deliver care when patient needs evolve
  • Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
  • Provide education to patients and family members involved in patient care
Requirements:
  • Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS)
  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to/from care sites and/or work locations
  • One (1) year of experience practicing as a Licensed Practical Nurse (LPN) in a home health or similar setting, preferred
