We are seeking board-certified nurse practitioners to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. As an in-home health care provider, you will have the opportunity to connect with people one-on-one, helping them re-engage with their health and well-being.
Key Responsibilities:- Conduct in-home visits to assess a person's overall health and well-being
- Identify potential barriers to care and develop strategies to overcome them
- Earn up to $2,500 to $4,000 per week, with opportunities for additional compensation
- Work flexible hours, with shifts available Monday through Sunday
