Position Overview:The Clinical Nurse independently executes the responsibilities of a registered nurse in direct patient care. Employs the nursing process to deliver care that is appropriate for various developmental stages.
Additionally, collaborates with other healthcare professionals to create a comprehensive care plan that addresses the needs of patients and their families.
Key Responsibilities:
- Accountable for prescribing, delegating, and coordinating patient care. Utilizes clinical judgment based on nursing expertise and evidence-based guidelines to assess the patient's condition and develop a suitable care plan aimed at enhancing comfort.
- Ensures that patient care adheres to safety, effectiveness, patient rights, and guest relations standards.
- Supervises the care provided by the patient care team; coordinates the overall care plan.
- Educates and facilitates learning for patients, families, and the care team, demonstrating sensitivity to diversity.
- Collaborates with physicians, families, and other healthcare professionals to formulate and implement an effective care plan that encourages contributions from all parties for optimal patient outcomes.
- Advocates for patients, addressing their concerns and assisting in resolving ethical and clinical issues.
- Diligently documents all patient interactions in the electronic health record system.
- Safeguards Patient Health Information (PHI) at all times.
- Complies with all infection prevention protocols (e.g., hand hygiene, waste disposal).
- Provides skilled and competent care to patients and families based on their identified needs.
- Delivers care with a team-oriented approach, emphasizing strong customer relations, sound clinical judgment, and decision-making based on evidence-based practices.
- Continuously assesses the patient's condition through ongoing evaluation and modifies treatment as necessary in collaboration with the healthcare team, including the patient and family.
- Maintains a comprehensive knowledge base and resources to effectively manage patient and family needs within healthcare and community systems.
WORK EXPERIENCE
- Minimum of 1 year of RN experience required.
- Prior experience in Urgent Care, Ambulatory/Clinic settings, Clinic Coordination, or Phone Triage required.
EDUCATION
- Bachelor's degree or equivalent experience required.
LICENSES AND CERTIFICATIONS
- Valid nursing license in the State of Texas required.
- BLS certification from the American Heart Association required.
