About the Role

We are seeking a skilled Certified Home Health Licensed Practical Nurse to join our team at connectRN. As a home health LPN, you will be responsible for providing high-quality patient care and creating personalized plans to achieve desired outcomes.

Salary and Benefits

You will earn an estimated $50,000 per year, with opportunities to increase your earnings through productivity incentives and bonuses. Job Responsibilities
  • Perform comprehensive patient assessments and collaborate with a multidisciplinary care team to develop individualized plans of care.
  • Maintain continuity of patient care by communicating information timely and effectively with all team members providing care to the patient.
  • Provide patient episode management per protocol.
  • Complete documentation timely and accurately according to industry standards, conventions, and guidelines, including OASIS assessments, subsequent visits, or other required documentation.
  • Educate patients, caregivers, and family on aftercare or ongoing care of the patient.
Requirements
  • Current and unencumbered RN license
  • 1+ year(s) of Certified Home Health experience as an RN
  • OASIS experience
  • Strong assessment, critical thinking, and communication skills
  • Current CPR certification
  • Comfortability using technical tools and software/EMR for visit documentation
  • Reliable transportation, current driver's license, and car insurance coverage
  • Proof of Immunization/Exemption for Flu and COVID-19
  • TB Test or Chest X-Ray
