Job Title: Home Health Physical Therapist

Vitality Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team. As a Home Health Physical Therapist, you will play a vital role in providing high-quality rehabilitative care to patients in the comfort of their own homes.

Job Summary:

We are looking for a dedicated and experienced Physical Therapist to work with our interdisciplinary team to provide comprehensive physical therapy services to patients in the home health setting. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering patient-centered care.

Responsibilities:
  • Conduct thorough assessments of patients' physical capabilities, medical histories, and functional limitations to develop individualized treatment plans.
  • Implement evidence-based physical therapy interventions to improve patients' mobility, strength, balance, coordination, and range of motion.
  • Educate patients and their caregivers on proper techniques for performing therapeutic exercises, using assistive devices, and promoting optimal physical function.
  • Monitor patients' progress, reassess goals, and modify treatment plans as necessary to ensure optimal outcomes and continuity of care.
  • Collaborate with the interdisciplinary team to coordinate patient care and promote holistic rehabilitation.
  • Maintain accurate and up-to-date documentation of patient evaluations, treatment sessions, progress notes, and discharge summaries.
  • Provide emotional support and encouragement to patients and their families, fostering a trusting and therapeutic relationship.
  • Stay current with advancements in physical therapy techniques, technologies, and research findings to enhance the quality of care provided.
  • Adhere to ethical principles and professional standards of conduct, ensuring confidentiality, respect for patient autonomy, and compliance with regulatory requirements.
Qualifications:
  • Graduate of an accredited physical therapy program with a Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) degree.
  • Valid state licensure as a Physical Therapist, with current CPR certification.
  • Prior experience in home health, geriatrics, or rehabilitative care preferred.
  • Strong clinical assessment and critical thinking skills, with the ability to develop and implement individualized treatment plans.
  • Excellent interpersonal and communication skills, with the ability to establish rapport with patients, caregivers, and interdisciplinary team members.
  • Compassionate, patient-centered approach to care, with a commitment to empowering patients to achieve their rehabilitation goals.
  • Ability to work independently and manage time effectively, prioritizing tasks and adapting to changing patient needs.
  • Proficiency in documentation software and electronic health records (EHR) systems.
  • Willingness to travel to patients' homes within designated service areas and maintain a valid driver's license and reliable transportation.
