Job Summary

Key Responsibilities
  • Conduct thorough intake interviews to assess consumer needs and develop personalized service plans.
  • Collaborate with the clinical team to implement and evaluate treatment plans, ensuring the highest level of care and support.
  • Lead consumers in recreational, educational, and therapeutic activities, promoting their physical and emotional well-being.
  • Maintain accurate and up-to-date consumer documentation, adhering to industry standards and regulations.
  • Manage a caseload of up to 30 cases, prioritizing tasks and ensuring timely interventions.
  • Participate in regular clinical team meetings to share knowledge, best practices, and consumer progress.
Requirements
  • Master's degree in a human services-related field, such as psychology, social work, or counseling.
  • Significant experience in the mental health field, with a focus on long-term care and rehabilitation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong analytical and problem-solving skills, with the capacity to adapt to changing situations and priorities.
